Please Note:
We are closed on the following holidays: New Years – Jan 1st, Memorial Day, July 4th, Labor Day, Thanksgiving,  December 24th, 25th – Christmas, December 31st, and New Years January 1st.

If your are less than 18 years old, a parent or guardian must come with you for the 1st appointment. If this is impossible, we will talk to you about a "court bypass" for this part of Ohio law
This is information you will need for your appointment:

1. Since you are making your appointment on our website, please bring $100.00 in cash, money order, or credit card. We do not accept checks. Credit cards are ONLY accepted by the owner of the card.
2. We have parking in the front and rear of our building.
3. You may bring a support person, but we do not have public rest rooms for them as space is limited . Additionally, only the patient is allowed beyond the lobby.
4. No children are allowed.
5. Expect to be with us for 2 - 3 hours.
6. We keep the clinic temperature at about 68º. Bring a jacket or sweater if this will be uncomfortable for you.
7. Please bring your Driver’s License or State Picture ID (Note: If a support person is needed, they need to provide a Driver's license or State Picture ID),
8. If you are under 18, your parent or guardian does not have to stay for the entire visit. We can have them out in about ½ hour. Your parent or guardian must bring their Driver’s License or State Picture ID with them.
